In my games I made each session count as one experience point. It costs 3 points to level up for levels 2-5, 4 points for levels 6-10 and 5 for levels 11-15. It's worked extremely well so far.

Toughest critter can also be a relative factor depending on how prepared the party is for the critter, or even their composition. Arzno, Merctown and Cuidad Juarez are all described in detail. I don't know if they count as fortified or not but, at least in the case of Merctown, you would have to have a lot of nerve to attack a city full of Mercs.

I was reading Ratty's guide to spell casting and at one point there is a discussion on how it's better to spend a couple rounds buffing vs. Casting damage spells. I 100% agree with this, at least in that it's better to cast something else or just use a gun and save your P.P.E., but I don't remember...

Missile rifle is on page 95 of coalition war campaign. It says it is standard issue for the Special Forces Striker SAMAS and frequently used by the Terror Trooper, full conversion borgs and the occasional Juicer. It does have a Strength limit. Oh, and the Striker has 28 mini-missiles of its own.
